I am looking for a tool that will show me:
Account lockout date/time-
last successfull login
last failed login
password age
last time password was changed
i worked for a company who had a tool that created an additional "view" in Active Directory that displayed that "Additional Information" related to Account Lockout...

Which tool should I download?

This tool will give you all the information you mentioned. Also with this tool you can reset password and modify all the Active Directory attributes.
